Inguinal Hernia TEP - Hernia Sac Ligation + No Baloon + No Mesh Fixation

Video demonstrate a videolaparoscopic hernioplasty with Total Extraperitoneal Technique, using permanent material. The patient had a direct + indirect hernia. The landmarks for the TEP surgery were highlighted on the video. The surgery starts withe the punction of the Retzus Space with Veres Needle, a prepneumoperitoneum was achived with 12mmHg of CO2. Afther that an umbilical incision was performed in the midline, rectus aponeurosis (anterior and posterior) were incised, and the preperitoneal space was achived, following by the introduction of a 10mm permanent trocar. Blunt dissection with a 30 degree scope until the supravesical space was achived, with the identification of the pubis and cooper ligament of both sides. After an extensive dissection and identification of all the landmarks, an indirect hernia sac was identified and ligated with prolene 0 external endoloop. At the end of the surgery a 15x12cm mesh was deployed over the posterior inguinal wall to cover all the possible hernias (direct, indirect and femoral, no fixation was used.
