Most symptomatic gallbladder diseases are operable by the new golden standard of Laparoscopic Cholecystectomy.There are very few instances that the old more invasive surgical approach of open cholecystectomy are required to remove the diseased gallbladder.
Gangrenous gallbladder with hardened/fibrosed surrounding tissues is once instance that are not amenable by Lap. Cholecystectomy.Nevertheless,once the patient is anesthetized,Laparoscopic Cholecystectomy is still attempted and once the initial dissection proves impossible to proceed,then the surgeon has to decide to convert to open cholecystectomy.
Therefore,Open Cholecystectomy is a natural progression of surgical maneuver when the laparoscopic approach meets the endpoint.Conversion to open technique should never be seen as a failure of laparoscopic approach......
