32 years old patient with primary infertility, h/o multiple open and laparoscopic surgery for treatment of advanced endometriosis, ovarian cyst and numerous IVE failure.
Laparoscopy revealed frozen pelvis, large complex (R) adnexal mass containing simple cyst, large teratoma and endometrioma, both ureters was severy adherent with DIE of pelvic floor, superficial rectal endometriosis and advanced Adenomyosis with multiple posterior adenomyomas.
(R) adnexa was not salvageable, removed, (L) adnexa treated conservativly, all DIE removed and adenomyomas resected off

腹腔镜透露冰冻骨盆（R），大型复杂附件包块含有简单的囊肿，大的畸胎瘤，子宫内膜异位症，与模具盆底，浅表直肠子宫内膜异位症和先进的多个后腺肌瘤与子宫腺肌病，，两侧输尿管塞弗里附着
（R）附属器官是不可挽救的，删除（L）附属器官conservativly对待，板牙和腺肌瘤切除
