Sleeve gastrectomy is demonstrated. The patient here is a 30 year old female, BMI 42 kg/m2.
The procedure starts with a TAP block to improve patient analgesia in the postoperative period. This a standard practice in the Bariatric unit. Next is the dissection along the greater curvature, from 4 cms from pyloroduodenal junction upto angle of His. Note the meticulous dissection and clearance of fat from the fundus.
The sleeve is fashioned loosely over a 33 Fr bougie. Staples are fired along the bougie, and specimen is extracted. Staple line is checked for leaks using methylene blue dye and air insufflation under water. No drains are inserted.
The patient is discharged within 24 hours of the surgery.
