Cholesteatoma in the left ear was operated using the inside out mastoidectomy technique. The head of the malleus was resected, the incus and stapes suprastructure were eroded. Type 1 fistula of the lateral semicircula canal was reconstructed at the last stage of the surgery after cleaning all the squamous epitelium around. Anterior eardrum remnant with the handle of the malleus was left and the cavity was closed with temporal fascia with the aim of second look surgery and hearing reconstruction using TORP.
