Dr. David Tuckman - This video shows a patient with a volar fracture dislocation of the proximal interphangeal joint. This can be a devastating injury to the joint with severe long term consequences. I performed a dorsal approach with a dorsal plating. Anatomic reduction of the joint can easily be achieved through this approach. A couple of pearls for the procedure:
-Develop the interval between the lateral bands and central slip
-Maintain the insertion of the central slip
-Subchondral position of the proximal screws best achieved with the finger in the lateral position under fluoroscopy
-Check screw length to ensure no penetration of the volar cortex
-Early range of motion |
