44 years old P3L3 ,110 kg body weight, k/c/o diabetes , controlled peri operatively on insulin, with c/o abnormal uterine bleeding with fibroid uterus was posted for TLH. Upon inserting a primary trocar supra umbilically, it was seen as omentum was adherent densely to the abdominal wall in such a way that it was forming a 'tent'.
Palmer's point was chosen to insert the camera and gradual adhesiolysis was done. Supraumbilical trocar was kept in situ as a marker. Soon it was followed by a hysterectomy.
