The patient had never cleaned this part over decades, so it developed inflammation and fibrous adhesion causing him pain and discomfort. These adhesion were released under local anesthesia and collected smegma was released. The inflammation caused by smegma over long period is the main cause of cancer occurring in this part of body so hygiene is very important.
