46 year old female with c/o abnormal uterine bleeding for 2 years. she was put on Mirena for 1.5 years but it did not work.
Total lap hysterectomy was done. It was a vertical previous CS scar on the abdomen.Uterus was smaller than normal size.Highly vascular uterus with unusual laterally adherent tissue.

Notice how purposefully manipulator has been perforated.
