Acute calculous cholecystitis is an absolute indication for the removal of the stone bearing gallbladder. Our preferred approach is to do the surgery ASAP after the diagnosis. In this 86 year old lady however, we had to wait 4 full days upon cessation of Plavix which is an anti-thrombotic agent. She was operated on day 5 without the use of any blood product. Argon laser was very effective to control the oozing from the liver bed. It is noteworthy that classical "artery" 1st approach was overlooked in this patient. |
