Video shows a very difficult laparoscopic cholecystectomy footage where partial ressection of a scleroatrophic gallbladder is performed. It was impossible do detach the infundibulum from the common bile duct due to intense fibrosis between those two structures. After the release of the fundus and the body of the gallbladder from liver bed, perforation of the posterior wal in the infundibulum area was observed and some biliar mud was present together with drainage of some pus from the fundus. At this moment, partial cholecystectomy, leaving the cystic duct area attached to the common bile duct, was performed, and the infundibulum was sutured on it's base with vicryl 5.0 "U" shape suture. 2 penrose drains were left in the hepatic recess for 5 days, no bile leak was observed. Patient discharged withou symptons after 6 days. |
