37 years old women with abdominal pain (on right hipocondrium) for 3 days (2 previous similar episodes before). Fisical examination showed descompression pain on lower right iliac fossa and right hipocondrium. Ultrasound and blood tests normal. CT scan showed a normal size appendix with a fecalith close to it's base, no further complications.
During the surgery: a retroceacal appendix was found with a normal base and a inflamed tip behind the right colonic angle.
Mesoappendix was cauterized with monoploar hook (starting from the base until the tip) and the base was ligated next to the colonic taenias with prolene endoloops. The specimen was extractec with handmade endobag.
Umbilical aponeurosis closed with prolene zero and skin with nylon 4.0.
Patient discharged to home next day from the surgery, without any pain without antibiotics. |
