Laparoscopic Heller myotomy + partial fundoplication remains the best possible treatment in achalasia provided that enough surgical expertise is available. Balloon dilatation is also a valid option in cases with increased surgical risk but it is noteworthy that dilatation is not free of complications & long term results are slightly worse than the operation. POEM on the other hand is still experimental & neither it's risks nor lomg term results are clearly established. Botox injections have very little value in few selected cases who are very poor candidates for the other alternatives outlined above. |
