A middle aged patient underwent snare resection of a large cecal polyp; pathology revealed villous adenoma. The patient came for endoscopic mucosal resection to avoid surgery. Notice that careful examination confirmed the suspicion of cancer and a few biopsies targeting the cancer focus confirmed the diagnosis. It is important to take time and examine the entire lesion before snare resection.When performing piecemeal EMR, this step is critical as one would want to attempt and resect a lesion completely and safely. If obvious cancer was not recognized and piecemeal resection was performed, the patient would end up requiring surgery for definitive treatment and at the same time has to go through the risks of EMR such as bleeding an d perforation.
