This is a case of severe acute appendicitis, retro-cecal and very high in position. These types of appendix needs meticulous and careful dissection. Laparoscopy is very suitable for these cases, where open approach requires extensions of incisions in most of them, and sometime it is very difficult to decide the direction of extention. A must see procedure.
