This is a case of cholelithiasis with mild chronic cholecystitis having some minor omental adhesion. The important matter to note here that, the liver is considerably enlarged and neck dissection is technically demanding. Also note that, high vascularity and bleeding during liver bed dissection should be taken into account with high patience. The way of proper hemostasis and putting a drain in a relatively dry situation is also important.
