The reasons to share this video are three: First it is not very infrequent to encounter a patient having elevated serum levels of liver enzymes which may suggest the presence of common bile duct stones. On the other hand; some of these patients are found to have nothing during an ERCP as the stones might already have passed to the intestine. Therefore MRCP is an invaluable non-invasive , non-radiating tool with very high sensitivity/specifity allowing better patient selection for ERCP. This case is a good example to that. Secondly; harmonic scalpel is a wonderful toy whenever present and can be easily used instead of cautery and it really diminishes bleeding as can be seen. Third; the so called "caterpillar hump" is very well demonstrated in this video.
