Laparoscopic fundoplication is the best mode of definitive therapy for patients having gastroesophageal reflux. Best fundoplication should secure reflux control and it should avoid functional side effects such as gas bloat and dysphagia. For this purpose posterior partial fundoplication (Toupet's) is the best alternative as shown by Prof Yerdel & many other specialists. During Toupet's the short gastrics are generally left alone but occasionally they need to be taken down. This case is an example to this necessity.
