Prof Roman Robotic assisted laparoscopic rectal shaving

The movie presents the robotic assisted laparoscopic procedure performed in a 47 ys old woman referred for deep endometriosis infiltrating the rectum, the vagina, and both uterosacral ligaments. She has complained severe pelvic pain during menses, deep dsypareunia and metrorrhages. Clinical examination and MRI revealed the infiltration of posterior vagina and high rectum, right ovarian endometrioma and huge adenomyosis. Astonishingly, she did not complain significant painful bowel movements.
I proposed her a surgical procedure sparing the rectum but including radical hysterectomy, large posterior vaginal excision, bilateral oophorectomy and rectal shaving. The operative time was 200 min, and postoperative outcomes were uneventful. Postoperative hormonal treatment by estradiol will be administered for several years.
