This is a laparoscopic right adrenalectomy that is being performed to remove a tumor or adenoma of the right adrenal gland. The right adrenal gland sits above the kidney, under the liver and adjacent to the vena cava. Consequently it takes a very careful and delicate dissection to remove it safely. Fortunately the laparoscopic techniques allows for removal of the gland without significant disruptions to the surrounding tissues. Consequently the patient is typically able to go home the next day.
In this video the liver is elevated and the gland is separated from the vena cava until the adrenal vein can be clipped and divided. This is a critical step in the case. Once this is done the gland can be elevated off of the kidney and small feeding adrenal arteries are divided. Once the gland is free it is placed into a bag and removed though one of the trocar sites. |
