This is a difficult penile implant placement done by Dr. Andrew C. Kramer at the University of Maryland in Baltimore- this is one of the highest volume sites in the world with the highest number of difficult revisions and 2nd opinions. Here I felt grafting is unnecessary and too invasive, and a semirigid implant with plaque excision is also unnecessary and too extreme.
The patient is young, extremely healthy, and has a dense plaque in his penis. The choices for surgical treatment and the surgical decision making is shown in this slightly edited video. The plaque is incised completely in order to safely and completely fill out the space of the penis. The goal is to break open the plaque, expand girth and preserve length.
