This video demonstrates a technique for performing endoscopically assisted extralevator abdominoperineal excision. It combines laparoscopic abdominal operation for the sigmoid and upper rectum with a transperineal "down to up" or "reverse" anorectal mobilisation including wide dissection into the levator ani using a Gelport Path device by Applied Medical. The enhanced optical view facilitates extralevator dissection and allows the patient to remain in lithothomy position throughout the operation.
