Patient: Middle aged woman with a large pedunculated polyp in the sigmoid colon, with the polyp head filling the lumen.
Comorbid conditions: Renal failure on dialysis, morbid obesity, antithrombotic therapy.
Anesthesia: Total intravenous anesthesia
Endoscope: Cap fitted pediatric colonoscope with CO2
Resection: Each step was shown from the beginning to the end without edits.
Step 1: Epinephrine injection into the head and stalk and base to create vasospasm and hemostasis.
Step 2: Endoscopic loop ligation
Step 3: Snare resection with coagulation current
Step 4: Retrieve the specimen and pin the stalk for pathology interpretation (as stalks can shrink); it helps pathologist slice the polyp parallel to the stalk.
Step 5: Clip the resection base above the loop for additional hemostasis. |
