Cholecystectomy - Huge Gallbladder + Slim Cystic Duct
65 years old diabetic, with cholecystitis dagnosed 2 weeks before, surgery was postponed becouse of clinical instabilization.
After stabilization, cholecystectomy was performed, and acute cholecistitits of a huge gallbladder with a very slim and long cystic duct was obeserd. An anterograde dissection was started until the infundibulum (it wasn't possible to push the fundus to expose the infundibulum). During the dissection of the infundibulum, the common hepatic duct was found very close to the cystic duct. At the end of the dissection, the cystic duct was ligated with 2 intracorporeal knots. Gallbladder was emptied with left hipocondrium 5mm trocar, and was inserted inside extraction glove. No considerable blood loss or cavity contamination observed.
