Advanced Right Colon Cancer - Lap Decompress Ileostomy
72 years old male patient stoped to eliminate flatus e feaces 15 days before the surgery. Nasogastric tube: fecaloid secretion in high volume. CT scan showed free fluid on the pelvis, lynph nodes on the right mesenterium and a mass on the lower right quadrant, Blood exam: anemia (Ht 28%, Hb 9g/dl), normal WBC, low albumine: 2g/dl.
Laparoscopy indicated to diagnose and if possible, perform an ileostomy.
During the surgery numerous implants were found in all quadrants, one of them was biopsied, a large tumor from ileoceacal valve were observed and adhered to the anterior wall of lower right quadrant.
laparoscopy assisted Ileostomy was then performed, with good post-operatory result. The nasogastric tube was removed next day from the surgery and the ileostomy started to work at full volume, 2 days after the surgery.
Patient is now recovering from the surgery, waiting the biopsy and will have a colonostopy soon to close the staging of his disease to decide if he will have quimioterapy before a new surgery to remove the tumor or not.
