Tips for sucessful removal of 5200 gram uterus through a hand assisted port

This video case presents tips for the removal of a 5200g uterus via hand–assisted total laparoscopic hysterectomy. Several advantages of the hand–assisted laparoscopic approach are demonstrated in this video which includes better visual exposure, tactile sensation, lysis of adhesion, rapid bleeding control, prevention of thermal injury and expedient morcellation. We also show numerous vessel sealing techniques with the 10mm LigaSure with the aide of a 45-degree angled laparoscope.

Conclusions: To date, this is the largest reported uterus to be removed laparoscopically. This case pushes
the boundaries of laparoscopic surgery.
Use of the hand-assisted port allows for safe removal of extremely large uteri by an experienced
laparoscopic surgeon, allowing for improved visualization during surgery, decreased blood loss, decreased
morcellation time, a shorter hospital stay and a quicker recovery for the patient compared to laparotomy.
