Transanal TME Proctectomy
Transanal total mesorectal excision (TaTME) is a technique that has the potential to give better views low in the pelvis helping the surgeon with more accurate dissection. This may result in better functional and oncological outcomes. TaTME also has the potential to reduce conversion to laparotomy and construct a more reliable single circular stapled anastomosis that might lower anastamotic leaks rates.
In this case, Greg Wynn and Ralph Austin from the ICENI Centre in Colchester, have performed a transanal minimally invasive surgery (TAMIS) proctectomy. Initial laparoscopy was performed to assess the pelvis for adhesions and suspend the sigmoid colon to the anterior abdominal wall to assist dissection from below. Subsequently, a parastomal hernia repair was performed with a collagen mesh inserted transanally (not shown here).
